Mobility & Balance: Peter Attia's Centenarian Decathlon — Working Backwards from 90

Most people plan their health forward from today. Attia argues we should plan backwards from where we want to be at 90.


In his book Outlive: The Science and Art of Longevity, physician and longevity researcher Peter Attia introduces a thought experiment that reframes how most people think about physical preparation for ageing.

The standard approach to fitness is present-focused: what do I need to do to be healthy now? Attia argues this is the wrong question. The right question is: what physical capabilities do I want to have at 80 or 90, and what do I need to build now to ensure I have them then?

The Marginal Decade Framework

Attia introduces what he calls the "marginal decade" — the last decade of life. Not because that's when health preparation should begin, but because it's the reference point from which all earlier preparation should be calibrated.

If you want to be able to climb a flight of stairs unassisted at 90, you need to be capable of climbing multiple flights at 70. If you want to lift a carry-on bag overhead at 80, you need to be carrying substantially more weight at 60. Physical capability declines at approximately 10–15% per decade after 40, accelerating after 70. Working backwards means accounting for that decline.

The Centenarian Decathlon

Attia asks patients to identify ten physical things they want to be able to do in the marginal decade. The specific list varies by person, but common entries include:

  • Get up from the floor without help
  • Carry grocery bags a reasonable distance
  • Lift a grandchild without pain
  • Hike on uneven terrain for several miles
  • Play with children or grandchildren actively
  • Travel and carry luggage independently
  • Climb stairs in buildings without lifts
  • Get in and out of a car with ease
  • Play a round of golf, or similar leisure activity
  • Live at home independently

These are not extraordinary ambitions. They're the basic functional capabilities that preserve dignity and independence in later life. And yet achieving them at 90 requires meaningfully greater capability at 60 and 70, given the trajectory of decline.

The Four Physical Pillars

Attia organises the physical preparation around four capacities:

Cardiorespiratory fitness (VO2 max): The single best predictor of longevity across populations. VO2 max declines approximately 10% per decade; maintaining it requires zone 2 training (sustained moderate-intensity aerobic work) and periodically pushing into VO2 max range (high-intensity intervals).

Strength: Muscle mass and functional strength are the primary determinants of whether someone can perform the centenarian decathlon at 90. Sarcopenia (age-related muscle loss) begins in the 30s and accelerates after 50. It is largely preventable with consistent resistance training. The specific muscles to prioritise: legs, posterior chain, grip.

Stability: The ability to apply force without losing control — balance, proprioception, and motor coordination. The SRT and single-leg balance test directly measure this. Falls prevention lives here.

Flexibility and mobility: Range of motion in hips, thoracic spine, and shoulders enables pain-free movement and reduces injury risk. Lost flexibility is recoverable, but requires consistent work.

Why This Matters Now, Not Later

The centenarian decathlon is a planning tool for people in their 30s, 40s, and 50s — not their 80s. The window to build the reserve you'll need later is the decade or two you're currently in.

The 100 Great Years perspective

The centenarian decathlon is 100 Great Years thinking made concrete. It asks not "what should I do for my health today?" but "what do I want to be capable of at 90, and what does that require of me now?" This is the planning horizon that distinguishes the 100 Great Years approach from conventional health advice. Physical capability does not have to decline on the standard trajectory. It can be maintained, and in many cases improved, well into the 70s and 80s — but only by people who started thinking about it early enough to build the reserve.
