Mental Health & Relationships: Stress, Cortisol, and Ageing — Why Mental Health Is a Physical Health Issue

There's no clean separation between mind and body. The same stress that keeps you up at night is driving inflammation, disrupting metabolism, and ageing your cells.


The idea that mental and physical health are separate domains — one treated by psychiatrists, the other by physicians — is a historical artefact. Modern biology has made the integrated nature of these systems undeniable. Chronic psychological stress produces measurable, lasting changes in the body. Those changes contribute directly to the major diseases of ageing.

The HPA Axis and Cortisol

When your brain perceives a threat — whether physical danger or an anxiety-inducing email — it activates the hypothalamic-pituitary-adrenal (HPA) axis, triggering the release of cortisol from the adrenal glands. Cortisol is acutely useful: it mobilises energy, sharpens focus, and prepares the body for action.

The problem with chronic stress is that the HPA axis never fully deactivates. Cortisol levels remain persistently elevated, and the body pays the price:

Systemic inflammation: Cortisol suppresses immune function in the short term (useful during acute stress) but drives chronic inflammation over time. Elevated inflammatory markers — including hsCRP — are consistently found in people with high perceived stress. This inflammation is the common pathway connecting chronic stress to cardiovascular disease, metabolic dysfunction, and certain cancers.

Insulin resistance: Cortisol raises blood glucose to provide energy for the perceived threat. Chronically elevated cortisol chronically raises blood glucose, driving the same metabolic deterioration as a poor diet.

Disrupted sleep: Cortisol and melatonin have an inverse relationship — high cortisol suppresses melatonin production. Chronic stress disrupts sleep architecture, reducing slow-wave and REM sleep, which in turn impairs HRV recovery and further elevates cortisol. The cycle is self-reinforcing.

Telomere shortening: Perhaps the most striking finding: chronic psychological stress accelerates telomere shortening — the erosion of the protective caps on chromosomes that acts as a biological clock for cellular ageing. A landmark study by Elissa Epel and colleagues found that mothers of chronically ill children showed telomere lengths equivalent to women 10 years older than their chronological age.

What This Means for the Biomarkers You're Tracking

Chronic stress is not just a mental health issue. It shows up in your blood:

  • Elevated hsCRP: Chronic stress drives systemic inflammation, measurable as elevated hsCRP
  • Rising HbA1c and fasting glucose: Cortisol impairs insulin sensitivity
  • Low HDL and high triglycerides: Stress-related metabolic disruption affects lipid metabolism
  • Suppressed vitamin D utilisation: Chronic inflammation impairs vitamin D receptor function

This is why the app tracks both biomarkers and psychological wellbeing: the two datasets are not independent. Persistent low wellbeing scores alongside elevated inflammatory markers tells a more complete story than either in isolation.

Effective Interventions

Regular aerobic exercise: The most consistent and potent reducer of resting cortisol and trait anxiety. 30+ minutes of moderate aerobic exercise 3–5 times per week produces measurable reductions in basal cortisol and inflammatory markers over 8–12 weeks.

Mindfulness-based stress reduction (MBSR): An 8-week structured mindfulness programme has the strongest evidence base of any psychological intervention for reducing inflammatory markers. Multiple RCTs show reductions in IL-6 and hsCRP following completion of MBSR.

Sleep quality: Addressing sleep is often the fastest lever for cortisol reduction. Sleep and cortisol have a bidirectional relationship — better sleep reduces cortisol; lower cortisol improves sleep.

Social support: As discussed in the Harvard Study article, close relationships buffer the cortisol response to stress. Having someone to talk to — genuinely talk to — is physiologically different from facing stress alone.

The 100 Great Years perspective

Chronic stress is expensive — not in the colloquial sense, but in the literal biological sense. Sustained cortisol elevation costs you in inflammation, insulin resistance, sleep quality, and telomere length. These are not abstract risks. They are the mechanisms through which stress accelerates every major disease of ageing. 100 Great Years tracks psychological wellbeing alongside physical biomarkers because the two are part of the same system. A high-performing health engine requires both a well-trained body and a well-recovered nervous system. Managing stress is not self-indulgence. It is maintenance.
