Stress & Recovery: Box Breathing and Physiological Sigh — Two Breathwork Techniques Backed by Science

You can change your physiological state in under two minutes using only your breath. Here's how — and why it works.


Of all the recovery tools available, breathwork is the most accessible: no equipment, no gym, no time block required. And yet the physiological effects are real, rapid, and measurable.

Two techniques in particular have strong evidence behind them.

Technique 1: Box Breathing

Box breathing (also called 4-4-4-4 breathing) was developed in military contexts for acute stress management but has since accumulated a solid evidence base for nervous system regulation.

The technique:

  • Inhale through the nose for 4 counts
  • Hold for 4 counts
  • Exhale slowly through the mouth for 4 counts
  • Hold for 4 counts
  • Repeat for 4–10 cycles (2–5 minutes)

Why it works: The extended exhale phase activates the vagus nerve, the primary pathway of the parasympathetic nervous system. The holds create a mild hypercapnia (elevated CO2), which signals the brainstem to reduce sympathetic activation. The result is measurable: heart rate slows, HRV increases, cortisol begins to fall.

A 2023 study published in Cell Reports Medicine (Balban et al., Stanford) directly compared three breathwork patterns against mindfulness meditation. All breathwork techniques produced superior acute stress reduction compared to mindfulness, with cyclic sighing showing the strongest effect. Box breathing was among the most effective for sustained calm.

Best used for: Pre-sleep wind-down, acute stress (before a difficult conversation or meeting), or any moment you notice sympathetic overactivation.

Technique 2: Physiological Sigh

The physiological sigh is a naturally occurring reflex — you've done it thousands of times without noticing. It's the double inhale through the nose followed by a long, slow exhale. Babies do it. Animals do it. Your body uses it automatically to re-inflate collapsed alveoli in the lungs and reset the CO2/O2 balance.

It also happens to be the fastest evidence-based technique for acute stress reduction.

The technique:

  • Inhale through the nose (normal depth)
  • Without exhaling, take a second short, sharp sniff through the nose
  • Exhale completely and slowly through the mouth — as long as possible

That's it. One cycle is sufficient for a measurable stress response. Repeat 5–10 times for stronger effect.

Why it works: The double inhale fully reinflates the alveoli, maximising the surface area for gas exchange. The long, slow exhale activates the vagal brake on the heart and produces a rapid HRV increase. The Balban 2023 study found cyclic sighing (repeated physiological sighs) produced the greatest reductions in anxiety and the largest improvements in mood of any breathwork technique tested.

Best used for: Immediate acute stress reduction — before a difficult moment, after receiving bad news, or when you notice your breathing has become shallow and fast.

A Note on Practice

Both techniques work on first use — you don't need to build a practice to get the benefit. That said, consistent practice does produce lasting effects: regular breathwork over weeks and months improves baseline HRV, reduces trait anxiety, and deepens the speed with which the relaxation response activates.

Five minutes of box breathing in the evening before sleep is a reasonable starting point. It costs nothing and the upside is real.
